Sound like your program?

The ones doing well aren't the problem.

It's the one who isn't — and there's no one place to go look. The documentation doesn't get done, and when it does it's three days late and half blank. Everybody's a 4, until the FTO tells you in a side email that they're not. Getting any data back out takes a week. So you clear someone you're not sure about, extend someone you can't justify, and when you finally have to let them go, there's nothing you'd put in front of HR.

We get it.

Six things have to work

It sounds like one problem. It's six, and each one fails on its own.

    The documentation doesn't get done

    The FTO is exhausted after a 12-hour shift with 16 calls. The DOR gets skipped, or written three days later from memory, half blank.

    What is done, nobody can get at

    It's in a paper binder in somebody's office, or a Google Sheet, or a form only one person knows how to open. There's no one place to go look.

    What comes back is inconsistent

    One FTO is brutal, another passes everyone. Nobody can prove which is which, so the ratings don't compare.

    And the standard itself is vague

    Most forms are "rate 1 to 5" with no definition of what a 3 means. So everybody's a 4, a gut feeling with a number on it, until the side email says they're not.

    Pulling it together takes a week

    To see how one trainee is doing, someone exports, prints to PDF, and summarizes by hand. Hours per person.

    And it still won't support a decision

    After all that, the file says "doing fine," which is nothing you'd put in front of HR when you need to prove someone wasn't.

And it feeds itself

More paperwork makes the record worse, not better.

“They over-complicate the FTO paperwork so much that my FTOs aren't teaching anymore. They're too busy doing paperwork.”

The way out isn't more documentation. It's fewer, better-shaped judgments.

Why it matters

All of it exists to serve three decisions.

Clear them

Release to independent practice. Get it wrong and an unready provider is alone with a patient, the failure nobody recovers from.

Extend them

More weeks of third-rider time. Real money, and someone has to justify the spend without looking arbitrary.

Terminate them

End a career. The decision that gets litigated, and the one the file almost never supports.

The same record has to hold up at all three.
